1. What the demo is
Before a healthcare practice or other organization signs up, Fieldstone Health, LLC, a Massachusetts limited liability company located at 14 Algonquian Drive, Natick, MA ("Fieldstone," "we" or "us"), may build one of its blank forms as a free demo so it can see how the form works. These Free Demo Terms apply to that demo. The person who accepts them does so for themselves and for the organization they represent ("you"), and confirms they are authorized to accept them for that organization. Our Website Terms of Use and Privacy Policy also apply.
2. Fictional information only
The demo is for fictional patient information and blank templates only. You will not:
- enter real patient information, or let anyone else do so;
- share the demo link with patients, publish it on a website, or otherwise make it available for patient use; or
- connect the demo to other systems.
3. How the demo works
- Every page and PDF is marked "Demo – not for patients."
- File uploads are turned off.
- The demo accepts up to 25 test submissions and expires 15 days after it is set up, unless your organization signs the Business Associate Agreement, service schedule and Services Agreement and we activate the service within that time; the form then stays yours and its demo markings come off.
- Test submissions go only to the person who requested the demo. You and Fieldstone can both see them.
- Fieldstone enforces these specific limits on its own systems, not only on the page. They can't recognize real patient information, which is why section 2 relies on you.
4. Going live
Using any form with real patients requires a signed Fieldstone Services Agreement, Business Associate Agreement and applicable service schedule, and Fieldstone's written activation of the service. Until then, the demo stays a demo.
5. If real patient information is entered
If real patient information is entered in a demo despite these terms, tell us right away at hello@secure-form.com. When we discover it, we restrict access, stop unnecessary processing, and assess whether it was accessed or disclosed improperly. We contact the requester or responsible organization as appropriate, give any notices the law or our agreements require, and arrange secure return or deletion. We may keep a limited, protected incident record where needed for compliance, and we may end the demo.
6. Ending the demo
The demo ends when it expires, when it reaches its submission limit, when your organization's service is activated, or when either of us ends it. We may end any demo at any time. When it ends, test submissions are deleted, backup copies expire within 30 days, and copies of demo emails kept by our email provider are deleted within 30 days, with its backups expiring within a further 30 days.
7. Ownership
You keep ownership of the template, wording and logo you send us, and let us use them to build and run the demo. We own the web form's design and code. The demo gives you no right to use the form after the demo ends unless your organization signs up.
8. No warranty; limits on liability
The demo is free and provided "as is," without warranties of any kind, to the extent the law allows. We are not liable for indirect, incidental, special, consequential or punitive damages arising from the demo, and our total liability for it is limited to $100, except where the law doesn't allow such limits.
9. Governing law
These terms are governed by the laws of the Commonwealth of Massachusetts. Subject to applicable nonwaivable law, any dispute about them must be brought in the state courts located in Middlesex County, Massachusetts or the United States District Court for the District of Massachusetts.
10. Acceptance
Before using the demo, the user must tick this box. It is not ticked in advance.
☐ I accept the Fieldstone Health Free Demo Terms for myself and for my organization, which I am authorized to represent. I understand this demo is for fictional patient information and blank forms only. I will not enter real patient information or share the demo for patient use. Real patient use requires a signed Fieldstone Health Business Associate Agreement, service schedule and Services Agreement, and Fieldstone's activation of the service.
Fieldstone records who accepted, the date and time, and the version of these terms.
